HEALTH ACKNOWLEDGMENT

Parents/guardians are responsible for ensuring their child is physically able to participate in Sínia Play activities. I understand that Sínia Play is a parent-supervised program and that I am responsible for supervising my child at all times.

PHOTO & VIDEO WAIVER

All clients attending Sínia Play sessions may be photographed or filmed. By participating, you grant Sínia Play, its legal representatives, partners, and assigns:

a) The unrestricted right and permission to copyright, use, reuse, publish, and republish photographs or videos of you and/or your child, intact or in part, composite or distorted, with or without names, through any media now or later known, for purposes including but not limited to illustration, promotion, advertising, and social media.

b) Permission to use any printed or digital materials connected to these images.

c) Waiver of any right to inspect or approve the final product, media, or advertising materials.

d) Release and discharge of Sínia Play, its representatives, and all persons acting under their permission from any liability arising from blurring, distortion, alterations, or composite use, whether intentional or otherwise, including claims for libel or invasion of privacy.

LIABILITY WAIVER

I agree to assume all risks associated with participation in Sínia Play activities, including but not limited to playdates, pop-ups, rentals, or special events. I understand these activities carry inherent risks such as minor injuries, environmental exposure, or equipment-related incidents.

I agree to hold Sínia Play, its owners, contractors, partners, and the venue harmless from any and all liability, claims, or demands arising from participation, except in cases of gross negligence or intentional misconduct as defined under California law.

ASSUMPTION OF RISK & RELEASE OF LIABILITY

I acknowledge that I have carefully read this waiver and fully understand that it is a release of liability and an express assumption of risk agreement. I understand that by agreeing, I am waiving certain legal rights, including the right to bring legal action against Sínia Play for ordinary negligence.

PARTICIPANT RELEASE

I affirm that I am at least 18 years old and the legal parent or guardian of the participating child. I freely agree to the terms above on behalf of myself and my child.
